Appropriate Use Agreement for Internet Databases The purpose of the interactive Oklahoma State Department of Health (OSDH) OK2SHARE Service databases is to support the information needs of the Oklahoma State Department of Health and other users such as health officials, educators, and students in improving service delivery, evaluating health care systems, and monitoring the health of the people of Oklahoma. These databases may be used only for the purpose for which they are provided and may not be used in any way that would violate the HIPAA Privacy Rule . Any effort to determine the identity of any reported cases, or to use the information for any purpose other than for statistical reporting and analysis, is prohibited. Therefore, as a condition for my access to data provided by the OK2SHARE Service over the Internet, I agree to:
I understand that failure to adhere to the above agreement will result in loss of access to OSDH Internet databases, and I may be subject to legal penalties. Any use, release, or publication of health data contrary to the provisions stated may be punishable by a fine of up to $5,000 (O.S. 63-1-120). If I am an Oklahoma state government employee, this may be grounds for immediate dismissal.
