The Discharge program collects 4 types of discharge data: 1) Inpatient discharges from hospitals, 2) Outpatient surgery discharges from hospitals, 3) Outpatient surgery discharges from free-standing ambulatory surgery centers (ASC), and 4) Emergency Department discharges from hospitals. Discharge data are collected in accordance with the Oklahoma Health Care Information System Act, defined in 63 O.S. (Supp. 1994) § 1-115 et seq. Oklahoma state licensed hospitals and ASCs are required to submit specific data related to patient discharges from their facilities. Data related to patient demographics, diagnoses, procedures, disposition and charges are reported.

Acute care hospitals are required to submit data on all inpatient discharges for a calendar year by May 1st of the following year. The Division of Health Care Information uses the UB04 billing format to collect inpatient discharge data. For each single inpatient hospital stay, a single discharge data record should be submitted. Discharge records should be submitted for persons discharged from all hospital beds, including acute medical/surgical care, swing, rehabilitation, psychiatric, and skilled nursing beds.

Acute care hospitals that perform outpatient surgeries are required to submit all outpatient surgery discharges for a calendar year by March 1st of the following year. The surgeries that are required by state are defined in the corresponding year‘s Data Submission ManualExternal Link. Hospitals use the UB04 format to collect data on hospital outpatient surgery.

All licensed free-standing ambulatory surgery centers are required to send in data on their outpatient surgery discharges for a calendar year by March 1st of the following year. The CMS1500 format is used to collect the data.
