Description:
Under the supervision of the Chief, Chronic Disease Service, this position will provide technical assistance and consultation in the areas of asthma, the chronic care model, and collaborative quality improvement. This position will serve as asthma health consultant. Technical assistance and consultation will be provided statewide in collaboration with healthcare, worksite, and community systems.
Duties include, but are not limited to:
This position will develop statewide program policy and procedure around asthma prevention and control, assure quality improvement and current medical/healthcare standards related to asthma, and provide technical assistance and consultation in the areas of asthma, and the chronic care model.
· Plan, organize, and manage the asthma program.
· Monitor, analyze, and evaluate the cooperative agreement and reports to assure goals, objectives and work performed are consistent with the asthma state plan of action and asthma burden statements.
· Provide technical assistance and consult with healthcare, business and community based systems to improve asthma burden among settings.
· Develop, consult on and present asthma related educational programs for professionals and non-professionals.
· Participate, consult and collaborate in the development of long-range plans for asthma.
· Participate in and consult on the development and monitoring of contracts or budgets as assigned.
· Establish and track quality improvement indicators related to asthma.
· Position requires possible response to public health emergencies including but not limited to, natural disasters, disease outbreaks, or catastrophic events.
Education and Work Experience:
Requirements at this level consist of a bachelor's degree in public health, healthcare administration, social sciences or a closely related field; or an equivalent combination of education and experience substituting one year of professional experience in one of the above fields for each thirty semester hours of the required education. Preference will be given to applicants with 1 or more years experience in managing budgets, staff, and the coordination of a statewide coalition.
Knowledge, Skills and Abilities:
Requirements for this position include but are not limited to: knowledge of public health problems, principles and practices; of theoretical concepts of health behavior and application of such; group organization and leadership; of community infrastructure development; current social and economic problems as related to public health; statistical methods; basic epidemiology; and grant preparation. Ability is required to establish and maintain effective working relationships with diverse partners and collaborate as an effective team member; communicate effectively, both orally and in writing; and exercise sound judgment. Applicant should possess good problem solving and leadership skills.
