interested in being considered for transfer and/or promotion
- Submit an ODH Form 92A
and an OPM 4-B (Personal Data Summary Sheet) that
clearly indicates how you meet the education and/or experience
requirements for the desired position by 5:00 p.m. on the
closing date of this announcement. Forms may be submitted

or by regular mail to Personnel Services, 1000 NE 10th Street,
Oklahoma City, OK 73117-1299.

by submitting a written request along with an OPM
4-B (Personal Data Summary Sheet) that clearly

DIRECT HIRE: For certain classified
positions requiring professional practice licensure or
those positions meeting approval criteria as hard-to-fill,
outside applicants may apply directly to the Department
of Health’s central office service, division or
county health department having a posted vacancy without
the need to be certified to the agency on a Merit System
list of eligibles (register). Applicants must
submit a completed Merit System Application Form (OPM-4)
showing they meet all minimum qualifications for the
position and level as stated in the job family descriptor
and attach a copy of their current professional practice
license if applicable for the position.

by the State Office of Personnel Management for the following
professional practice licensure positions:
Registered Nurse

Advance Practice Nurse (All Levels)
Clinical Health Facility Surveyor (All Levels)
Licensed Practical Nurse (All Levels)
Speech Language Pathologist (Levels II, III, IV)
Audiologist (Levels II, III)
Occupational Therapist (All Levels)
Physical Therapist (All Levels)
Nutrition Therapist (Levels II, III, IV)
Direct Hire Authority is authorized for the Department of Health
by the State Office of Personnel Management for the following
hard-to-fill positions:
Epidemiologist
(Levels II, III, IV)
UNCLASSIFIED JOBS
Qualified applicants interested in an announced unclassified
